Vital signs serve as an essential clinical compass in general internal medicine, providing immediate and objective data about a patient’s physiological state. The four most commonly measured vital signs are blood pressure, pulse, respiratory rate, and temperature. Each of these measurements offers distinct insights into the functioning of major body systems and collectively guides both rapid and long-term medical decision-making. Assessment of these four vital signs is a core component of patient evaluation in both emergency medical services and general internal medicine settings.
Blood pressure is a measurement of the force exerted by circulating blood on the walls of arteries. In clinical practice, the focus is on systemic arterial blood pressure, specifically in large arteries like the aorta or brachial artery. The measurement is expressed as two numbers: systolic pressure, which is the arterial pressure during ventricular contraction, and diastolic pressure, which is the pressure during ventricular relaxation. The typical normal value is considered to be 120 millimeters of mercury systolic over 80 millimeters diastolic. This is not a division, but rather two distinct measurements representing different phases of the cardiac cycle.
Pulse is often understood to include more than just heart rate. It encompasses the heart rate, rhythm, volume, character, and even the equality of pulse between different limbs. For example, in adults, the radial and carotid arteries are commonly used to assess the pulse, while in infants, the brachial artery is often easiest to palpate. The normal heart rate typically ranges from 60 to 90 beats per minute, with the rate determined by the ventricular contractions felt at the peripheral arteries. The rhythm of the pulse should be regular and predictable; deviations, such as irregularly irregular rhythms, may indicate arrhythmias like atrial fibrillation.
Respiratory rate, another vital sign, refers to how many breaths a person takes per minute. However, clinical assessment also considers the depth of respiration. Body temperature is the fourth commonly measured vital sign. It reflects the body’s ability to maintain internal stability despite external changes. Infections are a frequent cause of elevated temperature, or fever, which can also lead to associated findings such as leukocytosis and tachycardia. Bacterial infections, for example, release pyrogens that reset the hypothalamic thermostat, resulting in an increased core body temperature. Conversely, hypothermia, or abnormally low body temperature, can lead to metabolic acidosis, cell lysis, and even impair kidney function.
The distinction between signs and symptoms is foundational in clinical assessment. A sign is an objective finding detected by a healthcare provider, such as tenderness or a rash. For example, a provider may palpate a stiff, cord-like artery in a patient with atherosclerosis, which is an objective sign. In contrast, a symptom is a subjective experience reported by the patient, such as pain or headache. This distinction is not just academic; it directly influences the reliability and interpretation of clinical findings. For instance, while a patient’s complaint of chest pain is a symptom, the presence of an irregular pulse rhythm detected on examination is a sign.
The process of patient evaluation in both emergency settings and general internal medicine often begins with an assessment of the scene. Providers evaluate safety and context before proceeding to the patient’s airway, breathing, and circulation—the so-called ABCs. This approach ensures that life-threatening issues are addressed first. Once these basics are established, clinicians use vital signs to guide whether a patient requires immediate intervention or transportation, or whether a more conservative approach is appropriate.
Proper evaluation of vital signs directly informs the urgency of intervention. A patient with abnormal vital sign measurements—such as very low systolic blood pressure or a respiratory rate outside of the normal range—may require immediate stabilization and rapid transport to a higher level of care. Conversely, stable vital signs in a patient with minor complaints might allow for a “wait and treat” approach. The determination of patient priority, often referred to as triage, relies heavily on accurate and timely vital sign assessment.
For example, in emergency settings, a patient who is unresponsive and has no gag reflex, or a patient with severe dyspnea and shock, is typically prioritized for rapid transport and immediate intervention. In contrast, a patient with mild ankle pain or low-grade fever can often be managed on-site or observed over time. This is sometimes referred to as the “load and go” versus “wait and treat” or “stay and play” decision-making process.
Different approaches to patient management are tailored to the severity of the clinical presentation. The “wait and treat” method is applied to patients with minor conditions that can be managed with monitoring and supportive care. For example, in the case of stable ankle pain, the provider might choose to monitor symptoms and initiate treatment at the scene. On the other hand, the “load and go” approach is reserved for urgent or life-threatening cases. These include patients who cannot follow commands due to altered mental status, mothers in labor with complications, or those with severe pain, heavy bleeding, or signs of shock. The decision between these approaches is based on the rapid and systematic evaluation of vital signs, since these measurements reflect the body’s immediate response to illness or injury.
Each vital sign can provide specific clues to underlying pathology. High heart rate, or tachycardia, can be caused by fever, anxiety, exertion, medications like atropine and epinephrine, anemia, heart failure, myocardial infarction, or pulmonary embolism. Bradycardia, or slow heart rate, may result from medications like beta blockers, increased vagal tone, heart block, or during deep sleep. Blood pressure abnormalities are similarly informative: hypertension can result from increased heart rate, stroke volume, or peripheral vascular resistance, while hypotension may signal shock or fluid loss.
Respiratory rate changes are also diagnostically significant. Tachypnea, or rapid breathing, can result from exertion, stress, anxiety, medications such as aspirin, anemia, heart failure, myocardial infarction, or pulmonary embolism. Bradypnea, or slow breathing, may be seen in cases of opioid overdose, barbiturate use, diabetic ketoacidosis, or coma. Temperature fluctuations, whether fever or hypothermia, can be linked to infections, medications, environmental exposure, or systemic shock.
